Identity Number, date of birth, and gender are processed for the purposes of creating patient records, identity verification, and protection of public health, based on the legal grounds of "being explicitly provided for by the laws" and "fulfillment of the legal obligation of the data controller."
Your Contact Information
Your contact data such as address, phone number, and e-mail address are processed for the purposes of creating and confirming appointments, communicating about treatment processes, and billing, based on the legal grounds of "establishment or performance of a contract" and "legitimate interests of the data controller."
Your Health Information (Special Category Personal Data)
Your special category personal data such as medical history (anamnesis), complaints, diagnosis, treatment information, analysis and radiology results, prescription information, operation notes, and medical-purpose photographs are processed for the purposes of "conducting medical diagnosis, treatment, and care services" and preventive medicine, based on the legal ground of "processing by persons under the obligation of secrecy or authorized institutions and organizations for the purposes of protection of public health, preventive medicine, medical diagnosis, treatment and care services."
Your Financial Information
Your financial data such as bank account information, invoice, and payment details are processed for the purposes of collecting the fee for the health services provided, billing, and managing accounting processes, based on the legal grounds of "establishment or performance of a contract" and "fulfillment of the legal obligation of the data controller."
Your Digital Footprint Data
Data such as website usage data, cookie information, and IP address are processed for the purpose of improving the services offered via the website and enhancing the user experience, based on the legal ground of "legitimate interests of the data controller." 3. Parties to Whom Personal Data is Transferred and Purposes of Transfer
Your personal data may be shared with the following parties for the realization of the purposes mentioned above, within the framework of the conditions specified in Articles 8 and 9 of the KVKK:
- Authorized Public Institutions and Organizations: For the purpose of fulfilling legal notification and audit obligations with institutions such as the Ministry of Health, Social Security Institution (SGK).
- Contracted Health Institutions: With other health institutions collaborated with for medical diagnosis services such as analysis, laboratory, radiology, and the like.
- Business Partners and Suppliers: With service providers who provide support in areas such as accounting, law, information technologies (patient record software, website hosting, etc.) and who work under a confidentiality commitment.
- Insurance Companies: With the relevant insurance company for provision and payment processes if you receive services under private health insurance.
4. Method of Personal Data Collection
Your personal data is collected verbally during your personal application to our clinic, through patient consent forms and other written documents, and via channels such as contact and appointment forms on our website, e-mail, or phone calls.
5. Your Rights as a Data Subject (KVKK Article 11)
As a data subject, you can exercise the following rights by applying to our clinic, which is the Data Controller:
- To learn whether your personal data is being processed,
- To request information if your personal data has been processed,
- To learn the purpose of the processing and whether it is used in line with its purpose,
- To know the third parties to whom your personal data is transferred, domestically or abroad,
- To request the correction of your personal data if it is incomplete or incorrectly processed,
- To request the deletion or destruction of your personal data within the framework of the conditions stipulated in Article 7 of the KVKK,
- To request that the correction, deletion, or destruction operations be notified to the third parties to whom your personal data has been transferred,
- To object to the emergence of a result against you by analyzing the processed data exclusively through automated systems,
- To request compensation for damages in case you suffer damage due to the unlawful processing of your personal data.

You can submit your request to exercise your above-mentioned rights, along with documents verifying your identity, through the following methods:
